Here is some information about how to prepare for your office visit.
What to Bring
For your first office visit, please bring the following items with you:
- Current driver license or state identification
- Referral letter, if applicable
- Current insurance cards
- List of current medical conditions, medications and supplements
- Copies of any lab results, x-rays and other diagnostic tests
Patient Forms
The following patient forms need to be completed for your first office visit. You can download them before your appointment to save time when you arrive.
If you are unable to complete the forms on your own, you can have a friend or family member assist you. You only need to provide a signature once the forms are completed.
Insurance Information
Barbour Plastic Surgery accepts insurance coverage from most of the leading companies. We only accept insurance for our reconstructive services. Medical spa treatments and cosmetic surgery procedures are not covered by insurance.
You should understand your insurance policy prior to your first visit. You can contact the customer service number listed on your insurance card. You may have out-of-pocket expenses, such as:

- Co-insurance
If Barbour Plastic Surgery is considered out-of-network by your insurance carrier, your fees may be higher or the services may not be covered. We work with the following insurance companies:

Workers’ Compensation
Barbour Plastic Surgery works with individuals and employers for workers’ compensation cases that require reconstructive surgery. If you experienced an injury at work, discuss the coverage with your Human Resources Department prior to contacting us for an appointment.
